After choosing the icon style and background image, save the settings and it will update the homescreen.

You can add your custom icon in theme, you just need to add that icon in the respective icon set folder with it's menu name. Then re-select (change another theme, save it and reselct reselct the theme for which you have added the icon and save again) the icon set from settings.
